Frailty Virtual Ward Coordinator
OxFed is introducing new and improved ways of providing healthcare for older and frailer people living in Oxford. This includes setting up new Neighbourhood-based teams of health professionals to provide community-based care in and around the city. As most healthcare for frail people takes place in their own home, provided by their local general practice or community teams, we are developing a ‘Virtual Ward’ to help us better coordinate their treatment. This will allow us to monitor the progress and wellbeing of patients who are most in-need of care and support.
This new Virtual Ward requires a skilled Frailty Virtual Ward Coordinator so it can work effectively. As our Frailty Virtual Ward Coordinator , supporting the Virtual Ward in Oxford City will be one of your key responsibilities. You will work closely with a range of medical, nursing and other healthcare professionals, as well as having regular phone or email contact with patients and their carers. You will support the team with admin tasks, help coordinate care plans, liaise with a range of care providers, and keep important information about the patients accurate and up-to-date.
This is an evolving service that requires an adaptable coordinator with previous experience, in the healthcare environment. The role requires a responsible and conscientious individual who believes in the importance of improving the health and wellbeing of older people – and who has the flexibility to adapt to new and challenging situations. This will suit an enthusiastic and self-motivated person who enjoys being part of a busy team and wants to make a difference to the lives of some of the most vulnerable people living in our community.
