Specialist Clinical Pharmacist – Frailty Pathway including Care Homes
The holder of this post will be a pharmacist registered with the General Pharmaceutical Council, responsible for providing specialist prescribing and medicines optimisation support to frail elderly patients within the Oxford City locality including those residing in care homes. In discharging this responsibility, the post holder will provide expert advice, guidance, recommendations and any necessary training on medicines-related issues to care home staff, GP practice staff and members of the five neighbourhood teams delivering health and care services to the housebound population of Oxford City.
The pharmacist will conduct clinical medication reviews, working in partnership with key stakeholders including GPs, GP Clinical Pharmacists, care home staff, community services and acute trusts within a developing frailty pathway. The aim will be to optimise medication for individuals, improve prescribing quality and safety, support safe transfer of care between care settings, prevent hospital admissions and ensure cost effective and financially sustainable prescribing. The pharmacist will also lead for out of season provision of influenza prophylaxis and treatment in the care home settings in the Oxford City Locality, this will require them to be a prescriber.
The post holder will be a source of expert advice for stakeholders.
